Rafael R. Melendez, 37, passed away June 21, 2016 in Columbus, Ohio.
He was born on October 6, 1978 in Richland, Washington to Rafael and Gloria Melendez.
He is survived by his loving wife Nicole M. Melendez, 2 daughters, Lyric and Maddie Melendez and 2 step-children Jayde and Bryson, along with a host of other relatives.
Rafael will be greatly missed by family and friends.
To honor Richie's wishes the family hosted a private memorial to honor his memory.
